The Indiana Chronic Disease Management Program
offers nurse care managers to work with high-risk program members with
diabetes, CHF and pediatric asthma. Individual nurse assignments can be

Call Center
The Indiana Chronic Disease Management Program
offers a centralized Call Center for members of the program who are not
enrolled in case management. Quarterly educational calls are being made to
these members by the Call Center and they will also be mailing patient
