Note: To run CDMS you will need to download the MPQHF Enterprise Client. This will install the necessary libraries, images and support files. Once downloaded, launch installation of the MPQHF Client by double clicking on the file ChronicDiseaseManagementSystem(CDMS).exe . Follow the prompts. When you're done you should have a new program option, "MPQHF Enterprise Client," on your desktop.
